Non Refundable Deposit Fee & Policy
Your appointment has been successfully scheduled by your NON-REFUNDABLE deposit fee.
Your deposit fee is NON-REFUNDABLE and will be applied toward your date of service.
Please be advised of our Rescheduling Policy:
Each package includes a ONE TIME FREE rescheduling move, if you:
1) Reschedule your appointment 24 hours BEFORE your original date of service.
2) Reschedule your ORIGINAL booked package
3) Reschedule within 2 weeks of your original date
We will require a NEW $25, $50 or $100 rescheduling fee (based on your service) if you reschedule the same day of your appointment OR if you are rescheduling your appointment more than your one FREE rescheduling move.
*By rescheduling your appointment more than your FREE ONE TIME move, you grant us permission to use your card on file to secure your new date and time request with a $25, $50 or $100 rescheduling fee.
If you scheduled a specific package valid on specific dates, we will do our best to accommodate, however your original package choice may not be available.
On Time Arrivals
Please arrive at least 10 minutes before your scheduled appointment. If you are going to be late it is your job to give us a call to let you know you will be late. If you arrive more than 15 minutes late to your appointment, we cannot guarantee a scan and you are subject to the rescheduling fee.
Late Fee: I understand that I have been instructed to arrive 10 minutes before my scheduled appointment time. I am aware of the late policy and will communicate with Dutchess 3D Imaging, if I am running late. I am also aware that I may have to pay a $25 late fee and/or have to reschedule my appointment.
